If you're on Medicare and see a doctor who works with Companian, you're likely a good fit. Companian is built for older adults managing one or more ongoing health conditions who'd like a little extra support between doctor visits. The easiest way to find out is to ask your doctor whether Companian is available to you — that's always where it starts.
You're paired with your own Care Advocate: one consistent person who gets to know you and helps with the things that pile up between appointments. That includes transferring your medical records, scheduling and preparing for visits, keeping your medication list up to date, reviewing your bills and insurance statements, sorting out coverage questions, and connecting you to help like transportation or in-home care. Whenever something comes up, you have someone to call.
You enroll through your doctor. Because Companian works alongside the care you already get, your doctor introduces the program and refers you in — there's no signing up on your own. Just ask your doctor about Companian at your next visit. Once you're referred, your Care Advocate reaches out to get you started.
Your health information is protected under HIPAA and stays with your care team. Your Care Advocate calls you through a secure Companian line, and everything you discuss is kept in your private care record. Here in Florida, we'll always ask your permission before recording a call — nothing happens without you knowing.
Companian's support is provided under Medicare's covered care services, so for many people there's little to no out-of-pocket cost. Depending on your specific plan and any supplemental coverage, a small copay may apply. Your Care Advocate can walk you through exactly what to expect before anything begins.
